Faculty - Request information about your students' attendance at a Career Services Career Event

Whether you are requiring your students to attend an event or providing extra credit for their attendance, you can now request verification of their attendance.

One request is needed for each line number.  All fields above are required.

We will try to complete your request within 3-4 working days, and will provide the information in text format on a floppy disk.  There is a short transmittal form (text below) that you will need to sign when you receive the disk..

As you requested, I am delivering to you on the attached floppy disk the ASU student ID information for students in your ______ class who attended the Career Services _______ event.

Because this information is personally identifiable to ASU students, please observe the privacy protection rights outlined in the federal Family Educational Rights and Privacy Act of 1974.  Any reports released from this data should be in aggregate form only.

With the transfer of this information, you are accepting oversight of the data for its proper use and administration.
